Dr. Adam has been a long-time resident of Southern Wisconsin. He grew up in Baraboo and went to high school there. After graduation, he attended Hamline University in St. Paul, MN and received his bachelor’s degree and the University of Minnesota to complete his Dental School Training.
Dr. Matthew Clary grew up in Baraboo, Wisconsin. He attended the University of Wisconsin-La Crosse, where he received his undergraduate degree before graduating from Marquette School of Dentistry with his Doctor of Dental Surgery degree.
Dr. Marcin (Martin) Jasek grew up in Northern Illinois. He received his undergraduate degree as well as his Doctorate of Dental Surgery degree from Marquette University.

Very impressed everyone we've worked with have been fabulous! Dr Adam and the rest of the crew are very knowledgeable! They take time to listen to concerns and put you at ease with the care plan. We have a large family and always have a great experience regardless of who's turn it is for care. The kids are actually excited for the visits!!!
Everyone was very professional, from reception to assistant to dentist to billing. They answered all my questions. The fee was reasonable, and the explanation of charges was excellent. The process of making and fitting a crown was accomplished in one visit, and there were no problems after. They were all very friendly and downtoearth. Physically, it’s a wonderful office!

I switched to Prairie Lakes Dental two cleanings ago and I have been a big fan.
I got my first experience with getting fillings. I thought that it was going to really hurt and just be really painful, but it doesn’t hurt at all! I thank Dr. Forster and his assistant. It’s not as bad as you think!
Dr. Matt, I had a new bridge placed ...and thought the process was awesome. It did not take a long period of time due to having all the work completed in the office. I can’t explain how pleased I am with the treatment. The staff was very professional and nice.
They are super friendly and make you feel welcomed immediately. I had to have a couple of crowns done and they make them in house so you leave your appointment wearing your new crown. No longer a two trip process.
Dr. Forster, THANK YOU! My teeth were broken, infected, and painful for so many years. I had no opportunity financially to have them removed or to get dentures. Thanks to the Donated Dental Program and YOU,for removing them and fitting me with dentures. They fit well and are beautiful. Thanks to you – I am no longer ashamed to smile(which I now frequently do). Everyone at your office was helpful, professional, courteous and friendly. It has been truly a Miracle for me.
Just a little note to let all know that I had a good experience. Everyone is so kind and helpful, especially Dr. Forster who took care of my dental problems. He did the job he had to do with patience and thoughtfulness. You girls (assistants) are great too!!
